1. Your Rights and Responsibilities

Treatment Agreement

When you receive care at Embody Chiropractie BV, a medical treatment agreement is established under the Dutch Medical Treatment Agreement Act (WGBO). This law outlines both your rights and responsibilities as a patient.

Your Rights Include:

  • Access to your medical file.
  • Privacy and confidentiality of your medical data.
  • The right to refuse treatment at any time.
  • The right to receive clear information about your treatment.
  • The right to seek a second opinion.

Your Responsibilities Include:

  • Providing complete and accurate health information.
  • Following agreed-upon treatment plans and advice.
  • Making timely payments for services.
  • Providing accurate insurance information.
  • Keeping scheduled appointments or canceling with at least 24 hours’ notice.

2. Code of Conduct

To ensure a professional and respectful environment, all patients are expected to adhere to the following:

  • Treat all staff and other patients with courtesy and respect.
  • Refrain from any aggressive, abusive, or inappropriate behavior.
  • Follow clinic policies regarding appointments, cancellations, and payments.
  • Maintain personal hygiene and wear appropriate attire during visits.
  • Respect the privacy of other patients and avoid disruptive behavior.
  • Use mobile phones respectfully and keep them on silent mode during treatment.
  • Follow all health and safety guidelines as instructed by staff.

Failure to adhere to this Code of Conduct may result in a patient being asked to leave the clinic or having future appointments restricted.

3. Termination of Services

Embody Chiropractie BV reserves the right to discontinue services to any patient who:

  • Repeatedly fails to adhere to our appointment & cancellation policies.
  • Displays abusive, aggressive, or inappropriate behavior toward staff or other patients.
  • Refuses to comply with recommended treatment plans without valid reason.
  • Has an outstanding unpaid balance beyond the permitted timeframe.

If services are terminated, the patient will receive a written notice, and future appointments will no longer be scheduled.

4. Appointments & Cancellations

  • Appointments must be canceled or rescheduled at least 24 hours in advance.
  • Late cancellations or missed appointments may incur the full consultation fee.
  • All patients acknowledge this policy upon intake.
  • If you arrive late to an appointment but within your scheduled time slot, your session may be shortened to avoid inconvenience to other clients.
  • If you arrive after your scheduled time or miss the appointment entirely, the full consultation fee will apply.

5. Payment Terms

  • Payment is required at the time of service.
  • We offer pre-payment options for reduced costs.
  • Outstanding balances over 14 days:
    • May be subject to collection procedures.
    • May incur additional fees and interest charges as permitted by Dutch law.
    • May affect your ability to schedule future appointments.
  • Payment plans must be arranged in advance.
  • We provide invoices after payment for insurance submission.
  • Refunds are not possible for services rendered.

6. Insurance

  • Embody Chiropractie BV is a member of the Dutch Chiropractic Federation (DCF).
  • Patients are responsible for:
    • Submitting invoices to their insurance companies.
    • Understanding their own insurance coverage.
    • Verifying reimbursement eligibility.
  • Disclaimer: We do not guarantee reimbursement and are not responsible for verifying insurance policies on behalf of patients.

7. Intellectual Property Rights

  • All content on our website, marketing materials, and educational resources, including text, images, logos, and videos, are the intellectual property of Embody Chiropractie BV.
  • Unauthorized use, reproduction, distribution, or modification of any content without written permission is prohibited.
  • Any third-party content referenced or linked on our website belongs to their respective owners, and we do not claim ownership over external materials.

8. Online Content & Marketing Disclaimer

  • Any content published on our website, social media, or other digital platforms is for informational purposes only and does not constitute medical advice.
  • Chiropractic care is not a substitute for medical intervention, and individuals experiencing serious health conditions should consult with a medical professional.
  • Testimonials, reviews, or case studies presented on our website or marketing channels are individual experiences and do not guarantee similar outcomes for other patients.

9. Emergency Protocol

  • For medical emergencies outside of office hours, please contact your local emergency services.
  • Embody Chiropractie BV is not responsible for emergency or urgent care outside scheduled appointments.
  • We will inform patients if we believe their condition requires emergency medical attention.

10. Privacy and Data Protection

  • We process your personal data in compliance with GDPR regulations:

11. Force Majeure

  • Embody Chiropractie BV is not liable for any failure or delay in services due to events beyond our reasonable control, including but not limited to:
    • Natural disasters, pandemics, or public health crises.
    • Government regulations, changes in law, or professional association mandates.
    • Power outages, cyberattacks, or IT system failures impacting operations.
